Enhanced Primary Care Plans from Medicare

Did you know you could be eligible for fully funded sessions with an Accredited Exercise Physiologist through Medicare? It’s true, every eligible person is allowed 5 sessions in a calendar year.

What is an Enhanced Primary Care Plan?


An Enhanced Primary Care Plan (also known as a Chronic Disease Management Plan) is an initiative by Medicare that allows anyone with a chronic condition (6 months or longer), to access an allied health professional to help with the management or treatment of their condition. These professionals can include an Exercise Physiologist, audiologist, dietitian, occupational therapist and others.

Your conditions can include:

  • Lower back, neck, hip, knee, ankle pain
  • Cancer
  • Obesity
  • Diabetes
  • Arthritis
  • Poor balance
  • And many more…

How Do I Claim Eligibility For Allied Health Sessions?


Visit your GP and speak to them about an Enhanced Primary Care (EPC) Plan or Chronic Disease Management (CDM) Plan and get them to make it out to an Exercise Physiologist at EPA Health. Once you’ve done that, give us a call on 1300 869 169 and book in a session with us.


Do You Charge A Gap?


No, we do not!!! The referral we get from your doctor covers the entire cost of the session.


What do I Expect At My Subsidised Sessions?


In your first chronic disease management session, we will simply sit down and chat with you. We get to know what is going on with your body, what aggravates your conditions, your general health and establish some goals. If we have time, we complete a short assessment and see what we really need to work on.


In our second session, we go through a planned exercise session with specifically tailored exercises with you. We teach you the correct technique and ensure you are not doing anything to injure yourself further. We ensure you are using the correct muscles and feel it in the right places.

After this, you either complete your exercises during our group classes, which run for 30 minutes at $10 per class, or complete the exercises at home or your local gym. We like to catch up for a one on one session at least every 4 weeks to reassess your exercises and increase the intensity as your body would have gotten stronger.

So go see your doctor and talk to them about getting a care plan. Remember, you are only eligible for five sessions in a calendar year so if you don’t use them this year, you lose them.
